Bioidentical HRT Side Effects and Management – A 23MD Guide
A very important consideration for individuals undergoing hormone replacement therapy is to know that Bioidentical HRT is generally better tolerated, associated with fewer side effects, and is SAFER compared to non-bioidentical formulations. Understanding the side effects of HRT and their management is crucial for a safe and positive treatment experience.
Common Side Effects and Management Strategies
1. Headaches
Headaches can occur during the initial stages of HRT but typically subside as the body adjusts. Bioidentical hormones, due to their molecular similarity to the body’s natural hormones, are less likely to trigger headaches than synthetic versions. In fact, we have seen so many patients whose headaches have stopped or significantly reduced by simply using bioidentical HRT. Over-the-counter pain relievers like ibuprofen or paracetamol can provide relief is headaches are minor.
If headaches persist or are severe, consult your doctor to adjust the dosage or switch formulations.
2. Breast Tenderness or Pain
This discomfort usually improves within 4 to 6 weeks. Breast tenderness can be due to either the oestrogen or the progestogen. Bioidentical estrogen and progesterone has been reported to cause less breast tenderness compared to synthetic versions.
It may be partly or wholly alleviated by a low-fat, high-carbohydrate diet, without making any significant changes to the HRT. If this doesn’t work, your doctor may recommend
- Switching fro non-bioidentical estrogen to bioidentical estrogen
- Using a transdermal application of bioidentical estrogen
- Reducing the dosage of the estrogen ( bioidentical )
- Switching from non bioidentical progestins to bioidentical progesterone
- Reducing the dose of the progestogen, or the number of days per month that it is taken (from 14 to 10) can also be tried, but the dose should not go below that appropriate for the dose of oestrogen ( BMS guidance HRT).
- Consider vaginal micronised progesterone (off-label)
If you notice a lump or changes in your breasts, seek medical advice promptly.
3. Irregular Vaginal Bleeding or Spotting
Common in the first few months of continuous combined HRT, this usually settles within six months. Bioidentical progesterone is less likely to cause breakthrough bleeding than synthetic progestins. Higher doses of estrogen is more likely to cause unexpected bleeding than lower dosages. Using lower dosages of progesterone, or cyclical progesterone is also more to cause unexpected bleeding.
If bleeding occurs beyond the first 6 months of commencing HRT, or the bleeding is heavy, consult your doctor, as you will need to make adjustments to your dosages, and or investigate cause s for the bleeding.
4. Nausea
Nausea is more common with oral HRT but tends to diminish over time. Transdermal bioidentical estrogen (patches or creams) reduces the risk of nausea compared to oral estrogen. Taking HRT with food and opting for transdermal delivery can help alleviate nausea. If symptoms persist, discuss alternative formulations with your doctor.
5. Mood Changes
Some individuals experience mood swings or depressive symptoms during HRT. Bioidentical progesterone has been shown to have a more positive impact on mood and is less likely to contribute to depressive symptoms compared to synthetic progestins. If mood disturbances persist, your doctor may suggest adjustments to your regimen or additional support.
6. Fluid Retention and Bloating
HRT can cause fluid retention, leading to swelling in the hands, feet, or lower legs. Bioidentical progesterone has a mild diuretic effect, reducing the likelihood of bloating and fluid retention. Higher estrogen dosages are more likely to cause fluid retention than lower dosages. Engaging in regular physical activity and reducing salt intake can also help manage this side effect.
7. Weight Gain
Some individuals may experience weight gain due to increased appetite or fluid retention. Bioidentical hormone therapy is metabolised more efficiently by the body, making it less likely to contribute to excessive weight gain. Sometimes higher estrogen dosages can cause weight gain and water retention, so reducing the dosage often helps. Its important to note that too low a dose of estrogen can also cause weight gain. Maintaining a balanced diet and exercise routine is key. If these measures do not work, and BMI is raising to or above 27, you may want to discuss using a GLP2 agonist with your doctor.
8. Fatigue
Fatigue is a common symptom, particularly when starting HRT. Bioidentical hormones provide a more consistent hormonal balance, reducing the likelihood of energy fluctuations. Adjusting the dosage of estrogen can sometime help with lethargy, as too high or too low a dose can lead to fatigue. Prioritising good nutrition, regular exercise, and adequate sleep can also help combat fatigue.
